Over the past few weeks, you have assessed the health status of a population, and have identified at least one priority health issue within that population. This week, you watched videos and read articles that highlighted the need to engage community members to support meaningful involvement and leadership to develop, implement, and evaluate strategies to improve the health of the community and to eliminate racial and ethnic inequities in health.

Prompt questions:

1.Share a brief summary statement about the primary health concern you identified in your County Health Assessment.

2.Identify at least three specific organizations, groups or agencies that offer programs or policies in your county to improve the health condition or concern you identified. Hint: This may take some researching on your part–make a few phone calls, contact your discharge planner/social work department at your hospital, talk to someone at the community’s United Way offices or County Health Department. Then, contact the organizations themselves to learn about their programs/services, and how they are organized in partnerships in the community to achieve their common goals.

3.Describe the activities (programs, policies) that these three organizations offer that relate to the health concern or health promotion opportunity.

4.How do these organizations and individuals coordinate efforts with one another and engage community stakeholders to improve the health of their community?

◦Are there any structured partnerships or coalitions established for collaboration and collective action across multiple sectors of the community?

◦What is the common vision of this collective group?

◦How are partners and stakeholders engaged in the CHIP process?

◦How does the group measure success?

◦How might students or other nurses get involved in supporting these efforts?
